Top 15 Radiology Billing Companies in the USA

Top 15 Radiology Billing Companies in the USA

RadiologyBillingSolutions is our top pick for radiology billing services in the USA in 2026. It bills for radiology and nothing else, and its team tracks the things that actually change from state to state: the Medicare contractor’s coverage rules, how Medicaid is set up, and which outside company reviews imaging approvals. Zotec Partners, Ventra Health and Coronis Health are the other radiology-heavy national names here. The rest are broader billing companies that handle radiology as one specialty among several.

Introduction

Radiology billing in the U.S. isn’t one system. Medicare claims run through 12 regional contractors with their own coverage rules, every state runs Medicaid its own way, and most large insurers hand imaging approvals to one of three outside review companies.

That’s why a company that bills radiology well in one state can struggle in the next. We looked at 15 radiology billing companies that work nationally and judged them on radiology focus, the services they actually offer, and what they publish about their experience.

1. Radiology Billing Solutions (RBS)

Radiology Billing Solutions has billed exclusively for radiology since 2010 and works with practices across the country. Every coder on the team works imaging claims full-time, covering diagnostic CT, MRI, ultrasound and nuclear medicine along with interventional procedures, where bundled guidance codes and the 26/TC split catch generalist billers. The team runs the whole cycle in-house: coding and charge entry, prior authorization, denial management and appeals, A/R follow-up, and the ACR and IAC accreditation paperwork imaging centers need for Medicare. Clients include independent imaging centers, hospital radiology departments and interventional radiology groups, and the results are on the record: a 99% first-pass claim rate, a 25-day average A/R cycle and a 32% average reduction in denials.

In Business Since2010
What They HandleRadiology coding and charge entry, prior authorization, denial management and appeals, multi-state payer routing, A/R follow-up, ACR and IAC accreditation paperwork

Worth knowing before you read a denial letter

Most large insurers don't decide imaging requests themselves. They hand CT, MRI and PET approvals to one of three outside review companies, called radiology benefit managers, and which one you deal with depends on the plan.

eviCore

Owned by Cigna's Evernorth. Reviews imaging for Horizon Blue Cross Blue Shield of New Jersey, Health Net in California, Mercy Care in Arizona and CareFirst in Maryland, among others.

Carelon

Elevance Health's benefit manager, formerly AIM Specialty Health. Reviews advanced imaging for Anthem plans, including Anthem in New York (formerly Empire), and its guidelines sit behind Independence Blue Cross's imaging policy.

Evolent

Formerly National Imaging Associates, and the old NIA name still shows up on paperwork. Reviews imaging for Fidelis Care in New York, Sunshine Health and Florida Blue in Florida, Superior HealthPlan in Texas and Arizona Complete Health.

Same scan, three possible reviewers, and sometimes two different ones inside the same state. The denial letter tells you which company made the call, and that's where the appeal has to go. RadiologyBillingSolutions checks the reviewer on every denial before it writes the appeal.

How to Choose Radiology Billing Services in the USA

Don't start with price. A vendor that's a point cheaper but sends appeals to the wrong reviewer costs you more in lost claims than it saves in fees. Here's what we'd check:

  • Jurisdiction by jurisdiction. There are 12 Medicare contractor jurisdictions, each with its own coverage rules. Ask how the vendor tracks the ones your practice bills in.
  • Reviewer routing. Ask which outside reviewer each of your top five payers uses. If they can't answer on the spot, they'll be learning on your claims.
  • Real numbers. Clean claim rate, denial rate and days in A/R. Any vendor worth hiring can produce all three.
  • Radiology mechanics. They should handle the 26/TC split, NCCI bundling on imaging guidance and MIPPA accreditation rules for the technical component of MRI, CT and PET without a second thought.
  • Plain pricing. A flat percentage of collections, written into the contract, with no required software folded into the fee.

What Makes Radiology Billing Across the U.S. Hard to Get Right

Medicare Part A and B claims are processed by 12 regional contractors, each covering a group of states and each issuing its own Local Coverage Determinations. Texas runs through Novitas, California through Noridian, New York through National Government Services and Florida through First Coast. A multi-state group can face different documentation rules for the same CPT code depending on where the scan was done.

Medicaid looks different everywhere

Arizona runs nearly all Medicaid members through managed care. Georgia keeps a large fee-for-service population next to its managed care plans. California splits Medi-Cal into different managed care models county by county. A vendor that treats Medicaid as one program will misroute claims in most states.

Approval rules keep moving

A federal rule in effect since January 1, 2026 requires Medicare Advantage, Medicaid managed care and ACA marketplace plans to decide standard prior auth requests within seven calendar days and urgent ones within 72 hours, with a specific reason for any denial. Individual plans keep changing their own lists too: Highmark pulled more than 80 codes back from eviCore in January 2025 and took over all of its imaging approvals in December 2025, and Fidelis Care dropped prior auth for chest CT in June 2025. A billing team has to catch these as they happen.

Ready to Improve Your Radiology Collections?

Bottom Line: Radiology billing across the U.S. leaves little room for a generalist approach; 12 Medicare contractors with their own coverage rules, Medicaid programs that look different in every state, and three outside review companies deciding most imaging approvals. RadiologyBillingSolutions is structured specifically for this complexity: a radiology-exclusive service model where every coder, workflow, and payer rule update is dedicated to a single discipline. That focus is reflected in verifiable outcomes, including a 99% first-pass claim rate and a 25-day average A/R cycle, figures most competitors in this market do not publish. For practices evaluating radiology billing services in the USA, RadiologyBillingSolutions offers the clearest combination of specialization and documented performance on this list.
