RadiologyBillingSolutions is our top pick for radiology billing services in Illinois in 2026. It bills for radiology and nothing else, and it’s set up for a state where one Blue plan now sends imaging requests down three different review paths depending on the member’s coverage. Zotec Partners, Ventra Health and Coronis Health are the other radiology-heavy names here. The rest are broader billing companies that treat radiology as one specialty among several.
Illinois Medicare claims go through Jurisdiction 6, run by National Government Services, which also covers Minnesota and Wisconsin. NGS writes its own Local Coverage Determinations, so the documentation rules for a scan come from NGS, not a national default.
Medicaid is changing too. About 2.4 million of the state’s roughly 3.1 million Medicaid members are in HealthChoice Illinois plans, and in June 2026 the state picked six plans for the next contract: Aetna Better Health, Blue Cross and Blue Shield of Illinois, Humana, Meridian, Molina and, in Cook County only, CountyCare. Humana is new to the program, and the current contracts run through December 31, 2026. We looked at 11 radiology billing companies working in or near Illinois and judged them on how ready they are for both changes.
Radiology Billing Solutions has done one thing since 2010: bill for radiology. Its coders spend every day on imaging claims, from diagnostic CT, MRI, ultrasound and nuclear medicine to interventional work, where the 26/TC split and bundled guidance codes cause most of the trouble. Coding, charge entry, prior authorization, denial appeals, A/R follow-up and the ACR and IAC accreditation paperwork all stay with the same in-house team. Independent imaging centers, hospital radiology departments and interventional groups make up the client base, and the numbers are published: a 99% first-pass claim rate, a 25-day average A/R cycle and a 32% average reduction in denials.

| What They Handle | Radiology coding and charge entry, prior authorization, denial management and appeals, Medicaid plan routing, A/R follow-up, ACR and IAC accreditation paperwork |

Worth knowing before you read a denial letter
Many insurers don't decide imaging requests themselves. They hire outside companies, called radiology benefit managers, to review CT, MRI and PET requests. Blue Cross and Blue Shield of Illinois now uses a different route depending on the member's coverage.
Advanced imaging requests go to Carelon Medical Benefits Management, formerly AIM Specialty Health.
Imaging and cardiology requests moved from eviCore to Carelon in 2025, and first-level appeals moved from eviCore to BCBSIL itself.
Since January 1, 2026, BCBSIL reviews advanced imaging requests itself instead of sending them to eviCore.
One insurer, three review paths. The member's ID card tells you the plan; the denial letter tells you who actually made the decision, and that's where the appeal goes. RadiologyBillingSolutions checks both on every Illinois denial.
Skip the price sheet for now. A vendor that sends a BCBSIL Medicaid appeal to eviCore in 2026 will lose that claim no matter how cheap it is. Here's what we'd check:
National Government Services runs Jurisdiction 6 for Illinois, Minnesota and Wisconsin. Practices that also bill across the line in Indiana deal with a different contractor in Jurisdiction 8, so keep the two rule sets apart.
Every HealthChoice Illinois plan serves the whole state except CountyCare, which only enrolls members in Cook County. Meridian also runs YouthCare, a separate plan for youth in the state’s foster care system. Billing teams need to know which of these a patient is in before the claim goes out.
BCBSIL moved its Medicaid imaging reviews to Carelon in 2025 and pulled Medicare Advantage imaging in-house on January 1, 2026. A federal rule in effect since that same date also requires Medicare Advantage, Medicaid managed care and ACA marketplace plans to decide standard prior auth requests within seven calendar days and urgent ones within 72 hours, with a specific reason for any denial. Use those deadlines when an imaging request stalls.
Bottom Line: Illinois’s payer environment leaves little room for a generalist approach; one Blue plan with three imaging review paths, a Medicaid program heading into new contracts with a new plan in the mix, and NGS’s Jurisdiction 6 rules on the Medicare side. RadiologyBillingSolutions is structured specifically for this complexity: a radiology-exclusive service model where every coder, workflow, and Illinois payer rule update is dedicated to a single discipline. That focus is reflected in verifiable outcomes, including a 99% first-pass claim rate and a 25-day average A/R cycle, figures most competitors in this market do not publish. For practices evaluating radiology billing services in Illinois, RadiologyBillingSolutions offers the clearest combination of specialization and documented performance on this list.
